Wrists that hurt in a push-up, a plank or a downward dog. It is body weight on a hand at the end of the wrist's extension range, in a wrist that has neither the range nor the strength for it - a capacity problem rather than an injury.

How common: Very common in anyone starting bodyweight training, yoga or floor-based classes

What it is

A push-up or plank position puts the wrist at close to its maximum extension and then loads it with a large share of body weight. The joint surfaces at the back of the wrist compress, and the structures at the front are stretched under load.

Almost nobody trains the wrist for this. It is asked to do something at its end range, under high load, with no preparation - which is why the pain appears when someone starts a bodyweight or yoga programme rather than after an injury. Treated as a capacity problem it responds well.

What it feels like

  • Pain at the back of the wrist in push-ups, planks and downward dog
  • Fine for everything else, including gripping and lifting weights
  • Worse the longer the position is held
  • A pinching sensation at the crease rather than a diffuse ache
  • Aching for a while after the session and then settling completely

What causes it

Most people have more than one of these at once, and they feed each other. Read the list looking for the two or three that sound like your week rather than for the one true answer — the ones that describe you are the ones worth acting on.

End-range wrist extension under load
A flat-hand position puts the wrist at or beyond ninety degrees for many people. Loading a joint at its end range compresses the structures at the back, which is where the pinch comes from.
Insufficient wrist extension range
Many people simply do not have enough extension to reach a flat-hand position comfortably. The body finds the range by forcing the joint, and the joint objects.
Weak wrist and forearm musculature
The wrist has muscles that stabilise it under load and they are almost never trained. Grip strength is not the same thing as wrist stability in extension.
A sudden increase in floor work
Starting yoga, a bodyweight programme or a new class introduces many minutes of loaded wrist extension where there were previously none.
Poor hand position and technique
Hands too far forward, elbows flared, or weight concentrated on the heel of the hand all increase the extension angle and the load at the joint.
Bearing weight through a soft, unengaged hand
Actively gripping the floor and spreading the load across the whole hand reduces the pressure at the wrist crease considerably. Most people rest passively instead.
Body weight
The load through each hand is a substantial fraction of body weight, multiplied in a plank or a dynamic push-up. It is a genuine variable.
Previous wrist injury
A previous fracture or sprain often leaves reduced extension and residual sensitivity, both of which show up first in exactly this position.
Underlying wrist pathology - the exception
Ganglions, TFCC tears and carpal instability produce pain in the same positions but also with twisting, gripping and other loads. Pain that is not confined to the loaded-extension position deserves a proper look.

Who tends to get it

  • Anyone starting yoga, pilates or a bodyweight training programme
  • People with naturally limited wrist extension
  • Anyone with a previous wrist injury
  • People carrying more body weight, since the load per hand is higher
  • Anyone whose training never included direct wrist work, which is almost everyone

  • Long-held flat-hand positions before the range and strength are there
  • Increasing floor work volume quickly
  • Resting passively on the hand rather than actively gripping the floor
  • Pushing through sharp pinching pain, which irritates the joint
  • Only ever using a wrist wrap without building any capacity

Settles it

  • Reducing the extension angle - push-up handles, dumbbells, fists, or working on the forearms
  • Progressive wrist extension range work, done daily and gently
  • Wrist and forearm strengthening in the loaded position, built up gradually
  • Actively gripping the floor and spreading the load across the whole hand
  • Forearm massage and stretching, which reduces the tension pulling across the joint

What the evidence says: Progressive loaded wrist work solves this in weeks. The neutral-wrist workaround lets training continue meanwhile, which is what keeps people from quitting.

And whatever the body part: chest pain or unusual breathlessness on exertion; a fever alongside the pain; unexplained weight loss; pain that is constant and unrelated to how you move; new numbness, weakness or loss of bladder or bowel control; or one calf that is swollen, warm and painful. Any of those, stop and get assessed.

How it usually goes

This responds quickly once treated as a capacity issue - most people manage full flat-hand positions comfortably within six to twelve weeks of graded wrist loading, and the modifications make training possible in the meantime. Avoiding the position entirely keeps the capacity where it is, which is why the modifications are a bridge rather than a destination. Pain with gripping and twisting as well as with loading, or pain on the little-finger side of the wrist, is a different problem and worth assessing.
