Hours with the arms above shoulder height, which is a position the body can only hold briefly before it starts compensating through the neck and the low back. The cuff fatigues, the space narrows and everything above and below takes the difference.

How common: Shoulder disorders are among the highest-incidence occupational injuries in trades involving sustained overhead or arms-elevated work

What it is

Working overhead is one of the highest-demand sustained postures there is. The arm weighs several kilograms held out at the end of a long lever, the subacromial space narrows in elevation, and the rotator cuff and scapular muscles have to hold position continuously rather than intermittently.

The body can hold that for a few minutes. Over hours it compensates - arching the low back to gain reach, extending the neck to see the work, hitching the shoulder. Those compensations are where a substantial share of the symptoms come from, which is why overhead work produces neck and back problems as well as shoulder ones.

What it feels like

  • Aching shoulders building through the working day
  • Neck pain and headaches from looking up for hours
  • Low back ache from arching to reach
  • Arms fatiguing and needing to be dropped and shaken out
  • Difficulty sleeping on the shoulder, and stiffness the next morning

What causes it

Most people have more than one of these at once, and they feed each other. Read the list looking for the two or three that sound like your week rather than for the one true answer — the ones that describe you are the ones worth acting on.

Sustained elevation narrowing the subacromial space
The space where the cuff tendons run is narrowest with the arm elevated. Holding that position for hours compresses the tendons continuously rather than intermittently.
Rotator cuff and scapular fatigue
These muscles hold the joint centred and the blade rotated. They fatigue over a working day, and once they do the mechanics deteriorate and the compression worsens.
Holding arm weight at a long lever
An arm weighs several kilograms and is held at the end of a long moment arm. The load on the shoulder girdle is far higher than the weight of the tool suggests.
Neck extension to see the work
Looking up for hours loads the neck extensors continuously and compresses the upper cervical joints. It produces the headaches and the neck pain that accompany overhead trades.
Low back arching for reach
Gaining a few centimetres of reach by arching the low back loads the facet joints for hours a day. It is an unconscious compensation and a significant one.
Tool weight and vibration
Heavier tools multiply the load, and vibrating tools add their own effects on the hands, arms and shoulders.
No opportunity to vary the position
Duration without variation is the core problem. A ceiling does not move, so the position is fixed for as long as the job takes.
Ageing tendon
Cuff tendon quality declines from the forties. Tradespeople who managed the same work comfortably for twenty years find it becoming a problem in the same job.
No strength training
The cuff and scapular muscles are the ones the job depends on and almost nobody in the trades trains them. Capacity has to come from somewhere, and the work alone builds fatigue rather than strength.

  • Warehouse workers doing overhead stacking
  • Anyone over forty in these trades, when tendon quality declines
  • Workers with no strength training and no rotation between tasks

  • Raising the work or lowering yourself - scaffolding, platforms, extension tools - so the arms come down
  • Rotating tasks so overhead work is broken up rather than done in one block
  • Rotator cuff and scapular strengthening, which builds the capacity the job requires
  • Neck and thoracic mobility work to offset the sustained extension
  • Regular short breaks with the arms down, which restore blood flow to the working muscles

What the evidence says: Cuff and scapular endurance training reduces shoulder complaints in overhead workers. Lowering the working height does more than any exercise, so the routine should say that first.

Night shoulder pain, weakness lifting the arm, or arm numbness needs assessment before continuing the work pattern

And whatever the body part: chest pain or unusual breathlessness on exertion; a fever alongside the pain; unexplained weight loss; pain that is constant and unrelated to how you move; new numbness, weakness or loss of bladder or bowel control; or one calf that is swollen, warm and painful. Any of those, stop and get assessed.

How it usually goes

Strengthening the cuff and scapular muscles improves tolerance over eight to twelve weeks, and equipment changes that lower the work produce immediate relief. The exposure does not go away - it is the job - so this is ongoing maintenance rather than a course of treatment. Persistent shoulder pain, night pain, or weakness lifting the arm deserves proper assessment, since cuff tears become common in this population from the forties onward.
