The sudden vicious calf or foot cramp that wakes you at three in the morning, or seizes mid-exercise. It is a nerve excitability problem rather than a mineral deficiency, which is why the usual advice about salt and bananas so often does nothing.
How common: 35-60% of adults; up to 50% of over-60s report nocturnal cramps
What it is
A cramp is a sustained involuntary contraction driven by hyperexcitable motor neurons. The best-supported model is that the reflexes protecting the muscle from over-shortening become impaired, particularly when the muscle is already short and fatigued - which is exactly the state a calf is in under bedcovers with the foot pointed.
The dehydration and electrolyte explanation is deeply embedded and poorly supported for ordinary night cramps. It has some relevance in extreme endurance settings and in people on certain medications, but for most people the muscle's position and its fatigue state explain far more.
What it feels like
- A sudden violent seizing in the calf, foot arch or hamstring, usually at night
- The muscle visibly knotted and hard, and impossible to relax voluntarily
- Relief only when the muscle is forcibly lengthened
- Tenderness and residual soreness for a day or two afterwards
- During exercise, an unmistakeable warning tightening a few seconds before it locks
What causes it
Most people have more than one of these at once, and they feed each other. Read the
list looking for the two or three that sound like your week rather than for the one
true answer — the ones that describe you are the ones worth acting on.
- Motor neuron hyperexcitability
- The nerves supplying the muscle become over-ready to fire, and the reflexes that would normally shut a contraction down are less effective. This is the mechanism with the best evidence behind it, and it explains why stretching stops a cramp instantly.
- A shortened muscle at rest
- Lying with the foot pointed down under the weight of the covers holds the calf at its shortest for hours. A short muscle is far more prone to cramp than a lengthened one, which is why night cramps favour the calf.
- Fatigue from unaccustomed exercise
- A tired muscle cramps more readily. A long walk, a new activity or the first hot day of the year all raise the odds that night.
- Reduced ankle range and short calves
- Chronically short calves from heeled shoes, running or simple disuse sit closer to their cramping length all the time.
- Medications
- Diuretics, statins, some asthma inhalers, and several others are genuinely associated with cramps. If cramps started around a new prescription, that is worth raising rather than dismissing.
- Pregnancy
- Night cramps are extremely common in the second and third trimesters, driven by circulatory changes, weight, and altered mechanics. They are unpleasant and generally benign.
- Peripheral arterial disease or nerve compression
- Cramping calf pain that comes on reliably with walking and stops with rest is claudication, not an ordinary cramp - a circulation problem that needs assessing. Lumbar nerve compression can produce a similar pattern.
- Ageing
- Cramp frequency rises steeply with age, alongside motor unit loss and reduced flexibility. It is one of the commonest complaints in older adults and it is under-treated because it is assumed to be untreatable.
- Dehydration and electrolytes, in a narrower range than believed
- This matters in prolonged heat and endurance settings and in people with genuine electrolyte disturbance from illness or medication. It is not the explanation for most three-in-the-morning calf cramps.
Who tends to get it
- Older adults, in whom frequency rises sharply
- Pregnant women, particularly in later pregnancy
- Anyone on diuretics or statins
- People who have recently increased their walking, running or standing
- Anyone with short, stiff calves or reduced ankle range

- Sleeping with the foot pointed down under tight covers
- A sudden increase in activity, particularly in heat
- Sitting for long periods with the calves short
- Alcohol, which is associated with more frequent cramps
- Waiting for the cramp to pass rather than lengthening the muscle
Settles it
- Stretching the calf immediately, which stops a cramp within seconds - it is the reflex reset, not the lengthening, that does it
- Regular calf and hamstring stretching before bed, which has modest but real evidence for reducing frequency
- Keeping the bedcovers loose so the foot is not held pointed
- Massage and foam rolling of the calf, which reduces tone
- Building calf strength through range, so the muscle is working further from its cramping length

What the evidence says: Nightly calf and hamstring stretching cut cramp frequency and severity in a placebo-controlled RCT in over-55s. One of the cleanest wins on this list.

Cramps with weakness, wasting or twitching that persist need neurological review; new cramps on a statin or diuretic - check with prescriber
And whatever the body part: chest pain or unusual breathlessness on
exertion; a fever alongside the pain; unexplained weight loss; pain that is constant and
unrelated to how you move; new numbness, weakness or loss of bladder or bowel control; or
one calf that is swollen, warm and painful. Any of those, stop and get assessed.
How it usually goes
Nightly calf stretching reduces frequency and severity for many people within a few weeks, though it rarely abolishes cramps completely. The measures are cheap, safe and worth trying properly for a month before concluding they do not work. Cramping that is one-sided, that comes on predictably with walking and eases with rest, or that comes with weakness, numbness or colour change in the leg is a different problem and should be assessed rather than stretched.
