Neck and upper back ache in teenagers, driven by hours of sustained neck flexion over a phone during a period of rapid growth. The habits set now are the ones that produce a forty-year-old's neck.
How common: Neck pain in adolescents has roughly doubled with smartphone use; 30-50% report symptoms
What it is
Adolescents spend more hours a day looking down at screens than any generation before them, and they do it during the growth spurt - the period when the skeleton is changing fastest and soft tissue is temporarily short relative to it.
The evidence linking screen posture to adolescent neck pain is reasonably consistent, though the relationship is with duration and lack of variation rather than with posture itself. What matters most is that the habits and the muscular patterns forming now will be the baseline for the next fifty years.
What it feels like
- Aching across the top of the shoulders and the back of the neck
- Worse after long stretches on a phone, laptop or console
- Headaches at the base of the skull
- A visibly forward head and rounded shoulders
- Stiffness turning the head, and reduced tolerance for reading or study
What causes it
Most people have more than one of these at once, and they feed each other. Read the
list looking for the two or three that sound like your week rather than for the one
true answer — the ones that describe you are the ones worth acting on.
- Sustained neck flexion over a phone
- The load on the neck extensors rises steeply as the head tips forward. Hours a day in that position is a very large cumulative load on muscles that are meant to hold the head balanced, not carried.
- Duration without variation
- The consistent finding is that total screen time and unbroken duration matter more than the specific posture. A position held for four hours is the problem, whichever position it is.
- The growth spurt
- Rapid bone growth leaves soft tissue relatively short, and the nervous system's model of the body is temporarily out of date. Posture and coordination genuinely change during this period.
- Weak deep neck flexors and scapular muscles
- The muscles that would hold the head back and the shoulder blades down are almost never trained in adolescents, and they are the ones that fail first.
- Heavy school bags carried on one shoulder
- A one-sided load worn for a walk to school every day produces asymmetric loading of exactly these muscles.
- Low overall physical activity
- Adolescent activity levels have fallen substantially, and general strength is protective against postural aching in every study that has looked.
- Poor study and gaming setups
- Laptops on beds, phones in laps, consoles on low tables. The environment forces the position and no adolescent is going to overcome it by remembering to sit up.
- Sleep position and sleep duration
- Adolescents sleep less than they need and often in awkward positions with a phone. Both raise the muscular load and lower the threshold for pain.
- Being nagged rather than helped
- Telling a teenager to sit up straight does not work, because the position is produced by where the screen is. Moving the screen works.

- Adolescents with low overall physical activity
- Anyone studying or gaming on a laptop or phone rather than a set-up desk

- Breaks - the most effective single intervention, and easier to sell than a posture lecture
- Neck and upper back strengthening, which reduces symptoms in trials in this age group
- Two-strapped backpacks worn properly
- General physical activity, which is protective independently

And whatever the body part: chest pain or unusual breathlessness on
exertion; a fever alongside the pain; unexplained weight loss; pain that is constant and
unrelated to how you move; new numbness, weakness or loss of bladder or bowel control; or
one calf that is swollen, warm and painful. Any of those, stop and get assessed.
How it usually goes
Symptoms respond quickly to setup changes and breaks - usually within a few weeks - and strengthening builds tolerance over eight to twelve weeks. The longer-term value is different: the muscular capacity and the habits formed in adolescence set the baseline for adult neck health, and this is the easiest point in a life to influence them. Persistent neck pain in a teenager with night pain, weight loss or neurological symptoms needs assessment rather than a desk lamp.
