Tired, dry, aching eyes and a stiff neck after a day at a screen. Two problems arrive together: the eyes hold a sustained near-focus and blink less, while the neck and upper back hold a fixed position for hours.

What it is

Looking at something close requires the ciliary muscle inside the eye to contract continuously. Held for hours it fatigues, which produces the aching and blurring. At the same time blink rate drops by more than half when concentrating on a screen, so the tear film breaks up and the surface of the eye dries.

The postural half is a separate problem that shares a cause. A screen too low, too far or too close pulls the head forward and holds it there, and it is the neck and upper back that pay. Fixing one without the other usually leaves half the symptoms in place.

What it feels like

  • Tired, gritty or burning eyes by the afternoon
  • Blurring that clears when you look into the distance
  • Headache behind or around the eyes
  • Neck and upper back ache that arrives at the same time
  • Dryness that is worse in air conditioning or on a windy day

What causes it

Most people have more than one of these at once, and they feed each other. Read the list looking for the two or three that sound like your week rather than for the one true answer — the ones that describe you are the ones worth acting on.

Sustained near-focus
The muscle that focuses the lens works continuously at close range. Hours without a distance break fatigues it, which produces the ache and the intermittent blurring.
A halved blink rate
Blink rate falls dramatically during concentrated screen work, and incomplete blinks become common. The tear film evaporates, and the eye surface dries and stings.
Screen height and distance
A screen too low pulls the head down and forward; too close increases the focusing demand. A laptop on a desk is guaranteed to be too low, which is why an external monitor changes so much.
An uncorrected or outdated prescription
Squinting to resolve text recruits the muscles around the eyes and the forehead all day. Varifocals in particular can force a chin-up head position at a screen, adding a postural problem to an optical one.
Glare and poor lighting
Reflections and a screen much brighter or dimmer than its surroundings both increase effort. A screen in front of a bright window is one of the most common and most fixable causes.
Dry air
Air conditioning, central heating and fans all accelerate tear evaporation. The same screen time in a dry office produces far more symptoms.
Fixed head and neck position
Any position held for hours produces static muscle loading. The particular position matters less than the fact that it never changes.
Long unbroken sessions
Duration is the single biggest variable in every study of this. It is not that screens are harmful; it is that four hours without a break is.
Contact lenses
Lens wearers have measurably more dry eye symptoms during screen work, because the tear film is already compromised before the blink rate falls.

  • Anyone working at a screen for more than a few hours a day
  • Laptop users without an external monitor or a stand
  • Contact lens wearers
  • Anyone with an out-of-date prescription, or varifocal wearers at a screen
  • People working in air-conditioned or heated offices

  • A screen below eye level, which is the default with a laptop
  • Glare from a window or overhead light
  • Dry air from air conditioning or heating
  • Working through the symptoms, which is the usual response

Settles it

  • Looking at something twenty feet away for twenty seconds every twenty minutes, which relaxes the focusing muscle
  • Deliberate full blinks, which restore the tear film
  • Getting the top of the screen to roughly eye level and about an arm's length away
  • Neck and upper back strengthening, which addresses the postural half rather than only the eyes
  • Hourly movement breaks, which are the single best intervention for both halves

What the evidence says: Eye exercises do not fix refractive error. The neck and posture half of this is the part strength and stretching genuinely improve.

And whatever the body part: chest pain or unusual breathlessness on exertion; a fever alongside the pain; unexplained weight loss; pain that is constant and unrelated to how you move; new numbness, weakness or loss of bladder or bowel control; or one calf that is swollen, warm and painful. Any of those, stop and get assessed.

How it usually goes

The eye symptoms respond within days to breaks, blinking and screen position - this is one of the quickest wins on the whole site. The neck and upper back component takes longer, six to twelve weeks of strengthening alongside the setup change. Persistent blurring, double vision, eye pain rather than ache, or symptoms that do not settle overnight are not screen strain and need an optometrist or doctor rather than a break.
