Knees that pop on the stairs, a shoulder that clicks, knuckles that crack. Painless joint noise has no association with joint damage or with future arthritis - the noise is almost always gas, tendon or a normal roughened surface, and none of those is a warning.

How common: Almost universal; one of the commonest things people worry about and one of the least likely to matter

What it is

Most joint noise comes from one of three harmless sources. A sudden pressure drop in joint fluid forms a gas cavity that collapses with a pop - that is knuckle cracking, and it is why you cannot repeat it for twenty minutes. A tendon or ligament flicking over a bony ridge produces a snap. And normal age-related roughening of cartilage surfaces produces the fine grating of crepitus.

The evidence on this is unusually clear and unusually reassuring. Decades of study - including the famous case of a man who cracked the knuckles of one hand only for over sixty years - find no association between painless cracking and arthritis. The one thing that matters is whether it hurts.

What it feels like

  • A single sharp pop that cannot be repeated immediately
  • A snap or clunk as a tendon crosses a bone, often in the hip or shoulder
  • Fine grating or crunching in the knee on stairs or squatting
  • Noise on the first movement after sitting still, then silence
  • No pain, no swelling and no loss of function with any of it

What causes it

Most people have more than one of these at once, and they feed each other. Read the list looking for the two or three that sound like your week rather than for the one true answer — the ones that describe you are the ones worth acting on.

Cavitation in joint fluid
Stretching a joint drops the pressure inside it and dissolved gas forms a bubble which collapses audibly. It takes about twenty minutes for the gas to redissolve, which is why the same joint cannot be cracked twice in a row.
Tendons and ligaments flicking over bone
A tendon that has to cross a bony prominence can snap over it as the joint moves. The iliotibial band at the hip and the biceps tendon at the shoulder are the classic examples.
Normal cartilage surface roughening
Joint surfaces become slightly less smooth with age, which produces fine crepitus. It is present in enormous numbers of pain-free people and is a poor predictor of anything.
Air in soft tissue after movement
Small pockets of gas can move within tissue planes with movement, producing crackling that is entirely benign.
Kneecap tracking
The kneecap sliding slightly off its ideal groove can produce a click or a grind. If it is painless it is common and unimportant; if it hurts, the tracking is worth working on.
Hypermobility
People with more mobile joints get more noise, simply because their joints move through greater ranges and separate more easily.
Weakness around the joint
A poorly controlled joint moves less precisely, so structures catch and release more. This is why noise sometimes reduces as strength improves - not because anything was repaired.
Actual joint damage - the exception
A meniscal tear, a loose body or advanced arthritis can produce clicking or catching, but these come with pain, swelling, locking or giving way. The noise is the least important part of that picture.

Who tends to get it

  • Everyone - joint noise is close to universal and increases with age
  • People with hypermobile joints, who get considerably more of it
  • Anyone who has been inactive, in whom the first movements are noisiest
  • Adults over forty, in whom knee crepitus becomes very common
  • Habitual crackers, who are not doing themselves harm

  • Restricting movement because of the sound, which weakens the joint
  • Repeatedly forcing a joint to crack for relief, which trains the habit without changing anything
  • Long periods of inactivity, after which everything is noisier
  • Confusing painless noise with a symptom that needs investigating

Settles it

  • Regular movement, which reduces the first-movement noise most people notice
  • Strengthening around the joint, which improves control and often quietens it
  • Warming up before activity
  • Ignoring it, which is the correct treatment for the painless version
  • Getting it looked at if it hurts, catches, locks or swells - which is a different problem

And whatever the body part: chest pain or unusual breathlessness on exertion; a fever alongside the pain; unexplained weight loss; pain that is constant and unrelated to how you move; new numbness, weakness or loss of bladder or bowel control; or one calf that is swollen, warm and painful. Any of those, stop and get assessed.

How it usually goes

Painless joint noise does not need treating and does not progress to anything. It typically increases with age and with periods of inactivity, and often reduces when strength and regular movement improve. The line to watch is simple and worth remembering: noise alone is nothing, noise with pain, swelling, locking or giving way is worth assessing. Knuckle cracking specifically has been studied repeatedly and does not cause arthritis.
